Sel T CD4+ ini sangat berperan dalam respon immun. Pada penelitian terbaru di bidang genetik, rheumatoid arthritis sangat berhubungan dengan major-histocompatibility-complex class II antigen HLA-DRB1*0404 dan DRB1*0401. Fungsi utama dari molekul HLA class II adalah untuk mempresentasikan antigenic peptide kepada CD4+ sel T yang menujukkan bahwa rheumatoid arthritis disebabkan oleh arthritogenic yang belim teridentifikasi. Antigen ini bisa berupa antigen eksogen, seperti protein virus atau protein antigen endogen. Tubuh kita menemui kebutuhan peningkatan aktifitas dalam waktu yang lama, dengan aktifitas sedang dengan perbedaan respon fisiologis. Pada periode peningkatan aktifitas memunculkan respon tubuh dengan menghabiskan cadangan oksigen dan phospat dengan melalui pemecahan glikogen menjadi asam laktat. Keadaan ini menghasilkan defisit oksigen yang harus segera tergantikan. Penggunaan energi ini dapat digunakan dalam waktu yang singkat yang disebut dengan anaerobik power, jumlah toleransi defisit oksigen yang dapat diterima disebut dengan kapasitas anaerobik. Adaptasi dari kebutuhan dari peningkatan aktifitas pada jangka waktu pendek memiliki jumlah yang sama pada anak maupun orang dewasa.&lt;/span&gt;&lt;/p&gt; &lt;p class="MsoNormal"&gt;&lt;span&gt; &lt;/span&gt;&lt;/p&gt; &lt;p class="MsoNormal"&gt;&lt;span&gt;Pada waktu melakukan latihan akan terjadi perubahan jangka pendek. Ketika beban kerja meningkat konsumsi oksigen juga akan meningkat pada saat tersebut ambilan oksigen akan mencapai nilai maksimal. &lt;/span&gt;&lt;/p&gt; &lt;p&gt;&lt;span&gt; &lt;/span&gt;Respon Jangka Panjang&lt;/p&gt; &lt;p class="MsoNormal" style="text-align: justify; text-indent: 36pt;"&gt;&lt;span lang="SV"&gt;Tidak sama dengan latihan dalam jangka waktu yang pendek. Energi pada latihan dengan pemanasan diperoleh dari hasil proses oksidatif dari sumber makanan yang mulai muncul pada beberapa menit latihan dilakukan. Jumlah yang ditemukan dalam proses penyediaan energi dalam waktu lama dengan penggunaan oksigen dikenal dengan nama aerobik power. Penyediaan energi dalam latihan dengan pemanasan ini tergantung pada kesediaan oksigen dalam penggunaan kerja otot dalam waktu yang lama. Denyut nadi, frekwensi pernapasan, cardiac output, dan kebutuhan oksigen meningkat dalam latihan dalam waktu yang lama. Peningkatan frekwensi pernapasan akan meningkatkan jumlah oksigen dalam paru-paru yang akan meningkatkan proses difusi pada pembuluh darah. Peningkatan cardiac output akan meningkatkan jumlah darah yang ada pada pembuluh darah, akibatnya akan meningkatkan jumlah oksigen dalam otot. Dalam bagian penting peningkatkan cardiac output dapat diperoleh dengan adanya peningkatkan denyut nadi dan stroke volume. Perubahan stroke volume selama latihan relatif kecil, tapi salah satu keuntungan dari latihan adalah peningkatan stroke volume secara bermakna. Faktor penghambat dari aktifitas yang keras adalah kemampuan jantung sebagai pompa yang mampu mengirimkan darah dalam memenuhi kebutuhan oksigen ketika terjadi kerja otot. Pada kerja yang sangat berat peningkatan deyut nadi akan melewati batas kemampuan akhir dari aktifitas. Reaksi ; Fase Inflamsi . Fase pertama ini dapat terjadi sampai 72 jam. Diawali dengan proses perdarahan yang terjadi sampai 30 menit dimana pada saat itu jaringan yang trauma akan mengalami perdarahan karena pecahnya pembuluh darah. Setelah fase perdarahan maka akan muncul repon inflamasi yang dimanifestasikan dengan adanya nyeri, bengkak, panas. Jumlah cairan yang berlebih dan bengkak akan memulai proses perbaikan jaringan pada keaaan cidera dengan terjadinya pembentukan jaringan fibrin (bekas luka yang mengeras). Pada cidera otot dan tendon terjadi reaksi pemendekan myofilamen dan serabut otot pada dua jam pertama. Bengkak dan anoxia (jaringan tanpa suplai oksigen) akan menghsailkan kematian dan kerusakan sel pada 24 jam pertama. Setelah itu akan diikuti proses pelepasan protein dan pembentukan jaringan mati dan bengkak serta kekurangan oksogen pada jaringan&lt;/p&gt; &lt;p&gt;2. Regenerasi dan Reparasi; Fase fibroelastic repair. Fase ini terjadi mulai 48 jam sampai 6 minggu. Pada fase ini terjadi proses rebuild dan regenerasi jaringan. Fibroblast (bekas luka) tersintesa menjadi jaringan parut. Kemudian sel menghasilkan kolagen (jaringan ikat) tipe III, yang terjadi selama 4 hari. Perbaikan suply kapiler memproduksi kolagen yang menyilang (abnormal crosslink). Diakhir fase ini kontraktur dan pemendekan jaringan terjadi di area cidera&lt;/p&gt; &lt;p&gt;3. Fase remodeling. Fase ini terjadi mulai 3 minggu sampai 12 bulan. Pada fase ini jaringan kolagen yang menyilang dan memendek perlahan kembali menjadi jaringan yang sesuai dengan fungsinya. Seperti fungsi otot, tendon dan jaringan lainnya. Penyatuan, orientasi dan penyesuaian akhir dari jaringan terjadi pada fase ini.&lt;/p&gt; &lt;p&gt;Regenerasi jaringan setelah mengalami cidera sangat dipengaruhi oleh proses yang tepat dan baik pada setiap fase patologi jaringan. Salah dalam penanganan akan membuat jaringan akan sulit mencapai fungsinya yang optimal kembali. Selain itu banyaknya aliran darah yang teradapat pada jaringan juga akan mempersulit pengembalian fungsi jaringan. Oleh karena itu cidera otot yang banyak mengandung sirkulasi darah lebih sulit ke funsgi optimal daripada cidera bagian lain.&lt;/p&gt; &lt;/div&gt;         &lt;/div&gt;Pada saat jaringan mengalami trauma baik langsung maupun tidak langsung akan mengalami proses patologi jaringan. Proses patologi jaringan ini perlu diketahui untuk dapat memberikan penanganan yg tepat. Bila penangan yang diberikan tidak sesuai dengan proses patologi jaringannya maka akan menambah dan memperburuk cidera yang dialami. Memang sering terjadi cidera tersebut tidak terlalu membahayakan. Namun demikian ada beberapa faktor yang perlu menjadi perhatian yang menjadi prinsip dari pencegahan cidera pada olahraga. Bila prinsip ini dilaksanakan maka tubuh akan siap melakukan aktifitas olahraga dengan aman. Prinsip pencegahan cidera tersebut adalah&lt;/p&gt; &lt;ol&gt;&lt;li&gt;Kondisi fisik; adalah merupakan prinsip kunci dalam pencegahan cidera pada olahraga. Kondisi fisik yang baik akan mencegah terjadinya cidera pada waktu melakukan aktifitas olahraga. Juga akan mengurangi keparahan apabila mendapatkan cidera. Kemampuan maksimal dari penampilan seorang olahragawan akan diperoleh dengan kecukupan dalam kekuatan otot dan keseimbangan, power, daya tahan, kordinasi neuromuskuler, fleksibilitas sendi, daya tahan kardiovaskuler, dan komposisi tubuh yang sesuai untuk olahraga&lt;/li&gt;&lt;li&gt;Pemilihan metode latihan yang tepat; Pemilihan metode yang tepat adalah meliputi efisiensi gerakan yang sesuai, efketifitas program latihan, termasuk FITT (frekwensi, Intensitas, Time, Tipe) yang adekuat. Gerakan yang salah harus dikoreksi dan dengan dasar gerakan yang baik.&lt;/li&gt;&lt;li&gt;Rest dan recovery; tidur yang cukup adalah prinsip penting untuk mental yang baik dan kesehatan fisik serta menjadi bagian kritis dari recovery setelah bekerja berat. Kronik overexertion dan kelelahan dapat membuat atlet lebih mudah mengalami cidera&lt;/li&gt;&lt;li&gt;Muscle soreness; penggunaan otot yang diatas ambang kemampuan dapat memunculkan muscle soreness (luka pada otot), kekakuan otot, dan spasme otot. Berdasarkan hipotesa spasme otot pada luka pada otot, ischemic pada otot dan adanya nyeri pada otot menghasilkan reflek spastik kontraksi dan dilanjutkan dengan viscous circle ischemic, spasme dan nyeri. Sedang didasari oleh hipotesa kerusakan jaringan, microtear dan nyeri menghasilkan rangsangan pada nerve ending sehingga menimbulkan bengkak pada otot. Bengkak, spasme dan nyeri menandai adanya luka pada otot. Pemberian massage yang tepat akan dapat mengurangi bengkak (oedema), dan menurunkan spasme otot. Pemberian es dapat memfasilitasi proses penyembuhan luka, Dan istirahat yang cukup dapat menghindari kerusakan mikroskopik dari jaringan&lt;/li&gt;&lt;li&gt;Peralatan yang sesuai ; Sepatu merupakan peralatan yang paling harus disesuaikan dalam setiap aktifitas olahraga . Penggunaan sepatu ini sangat individualis dan harus dipilih secara hati hati. Cidera yang paling sering terjadi adalah pada ankle atau pergelangan kaki. Pada waktu terjadi cidera sebenarnya telah terjadi proses perdarahan seperti darah yang keluar pada luka. Cidera pada jaringan lunak seperti cidera ligamen, kapsul sendi, atau otot dapat terjadi baik oleh trauma langsung maupun tidak langsung.  Biasanya cidera jaringan lunak tersebut dihasilkan dari trauma tumpul atau beban yang berlebihan , keadaan ini dikenal dengan nama macrotrauma misalnya robekan otot atau sprain ligamen. Disisi lain trauma tidak langsung dihasilkan dari beban submaksimal yang disertai dengan tanda dan gejala dan tidak muncul secara tiba tiba.&lt;/p&gt; &lt;p&gt;Cidera sendiri terdiri dari 3 fase yaitu akut, subakut/overuse dan akut/kronis. Pada akut adalah fase trauma langsung dari beban berlebihan secara tiba tiba atau makrotrauma (misal gerakan meledak pelari 100 meter dari balok start). Fase subakut terjadi pada saat peningkatan beban degenerasi (proses penurunan anatomi dan fisiologi jaringan) pada jaringan tubuh yang terjadi secara kumulatif (contoh tendinitis achiles pada pelari jarak jauh).  Tipe terakhir adalah fase akut/kronik, adalah gabungan antara beban yang kumulatif dan beban berlebih secara tiba tiba (putusnya kronik tendinitis achiles pada pelompat jauh. Pada kronik sendiri adalah kondisi tanpa adanya inflamasi. Dan kondisi kronik ini akan menjadi akut yang disertai inflamasi bila mendapatkan beban berlebihan secara tiba tiba.&lt;/p&gt; &lt;p&gt;Inflamasi sendiri adalah reaksi radang yang ditandai adanya kemrahan, panas, nyeri, bengkak dan hilangnya fungsi. Inflamasi ini adalah tanda adanya kerusakan jaringan. Apakah cidera tersebut dihasilkan oleh trauma langsung atau tidak langsung hasilnya sama yaitu disfungsi karakteristik jaringan karena nyeri, inflamsi, dan stres pada jaringan ikat bagian dalam. Cidera sering menghasilkan ketidak mampuan fungsi, dimana pada saat itu atlet dapat melakukan aktifitasnya sehari hari tapi tidak dapat melakukan latihannya dengan baik.&lt;/p&gt;&lt;/div&gt; &lt;/div&gt; Tes ini dilakukan dengan cara pasien mengepalkan tangannya dimana ibujari diliputi oleh jari-jari lainnyas elanjutnya dilakukan deviasi ulner plus ekstension. Pengaplikasian paraffin-bath atau hot pack membantu mengurangi nyeri yang terjadi, karena dengan efek termal yang terjadi membantu meningkatkan proses vaskularisasi darah pada sendi. Berikut ini klasifikasi nyeri bahu dan pemeriksaan fisioterapinya :&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;a. &lt;strong&gt;Tendinitis Bicipitalis&lt;/strong&gt;&lt;br /&gt;- Adduksi bahu terbatas&lt;br /&gt;- Nyeri tekan pada tendon biceps (pada sulkus bisipitalis/sulkus intertuberkularis).&lt;br /&gt;- Yergason Tes + &amp;amp; timbul nyeri serta penonjolan tendon bicep&lt;/p&gt; &lt;p&gt;b. &lt;strong&gt;Ruptur Rotator Cuff&lt;/strong&gt;&lt;br /&gt;- Penderita dapat melakukan abduksi sampai 90 derajat, namun bila diminta meneruskan abduksi tersebut (elevasi),&lt;br /&gt;- Pada pemeriksaan kekuatan otot (MMI), nilai kekuatan otot tidak akan lebih dari 3 (Fair).&lt;br /&gt;- Gerak pasif biasanya tidak menimbulkan rasa nyeri, juga tidak ada gangguan.&lt;br /&gt;- Tes “Moseley” atau tes “lengan jauh” akan menunjukkan hasil yang positif.&lt;/p&gt; &lt;p&gt;c.&lt;strong&gt; Tendinitis Supraspinatus&lt;/strong&gt;&lt;br /&gt;- Painfull arc supraspinatus 0-60 derajat&lt;br /&gt;- Keterbatasan gerak sendi bahu, terutama abduksi dan eksorotasi.&lt;br /&gt;-  Nyeri tekan pada daerah tendon otot supraspinatus.&lt;br /&gt;- Tes “Apley Scratch” dan “Mosley”: positif. (Kedua tes ini bukan merupakan tes khusus bagi tendinitis supraspinatus saja. Tes “Apley Scratch” sedang tes dan “Mosley” juga positif pada kerusakan otot “rotator cuff” yang lain).&lt;/p&gt; &lt;p&gt;d. &lt;strong&gt;Bursitis&lt;/strong&gt;&lt;br /&gt;- Painfull arc sub acromialis 70 – 120 derajat.&lt;br /&gt;- Tes flexi siku melawan tahanan pada posisi flexi 90 derajat menjadikan rasa nyeri.&lt;/p&gt; &lt;p&gt;e. &lt;strong&gt;Capsulitis Adhesiva&lt;/strong&gt;&lt;br /&gt;- Adanya keterbatasan luas gerak sendi glenohumeral yang nyata, baik gerakan aktif maupun pasif.&lt;br /&gt;- Keadaan ini biasanya unilateral, terjadi pada usia antara 45 – 60 tahun dan lebih sering pada wanita.&lt;br /&gt;- Nyeri dirasakan pada daerah otot deltoideus.
